Licensed Practical Nurse - Assertive Community Treatment Program (ACT)
Full-time: Monday-Friday 9-5
Salary: $35-37/hour
Based in Glen Cove, NY
The Charles Evans Center (CEC) is a leading not-for-profit organization located in Nassau County. CEC serves adolescents and adults with addiction and behavioral health needs. We are currently seeking a few exceptional professionals who have a passion to create a meaningful impact in the lives of the amazing people we support.
We have a great opportunity for a positive, motivated individual to work in a multi-disciplinary, mobile outpatient OMH program with a specialty in health and self-wellness activities and initiatives for ACT recipients.
In this position, you will:
- Work with recipients to develop skills that will support their recovery and attainment of their goals
- Assist ACT medical staff with health-related activities when needed and appropriate
- Develop interventions based on OMH's Wellness Self-Management curriculum and other wellness resources
- Provide mobile community-based services
We require:
- Current NYS LPN license
- Clean, valid NYS driver's license required
- Must have transportation and be able to travel through Nassau and Suffolk County
- At least one year of training and/or experience in health and wellness activities for behavioral health recipients, preferred
