ACM Home Health is seeking to hire a Full Time RN Case Manager with a focus on Brown and Highland Counties.

Position responsibilities may include, but are not limited to:
- Completes and oversees Patient assessment and Plan of Care.
- Manages communication with physician and interdisciplinary care team consisting to ensure coordination of patient care and maximize patient outcomes
- Provide wound care, medication management, medication administration and other treatments as per physician’s orders.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Thorough documentation of all education and care provided through EMR.
Local travel is required so you must have reliable transportation and be comfortable driving in all weather conditions. Mileage is reimbursed.
Qualifications
- Active Ohio RN License (Required)
- Skilled Home Health experience (Preferred)
- OASIS Patient assessment skills
- Discharge planning expertise
- Accredited Case Manager certification
- Strong communication skills
- Ability to create effective care plans
- Proficient in time management
- Ability to work independently
