What are the responsibilities and job description for the Patient Centered Care Coordinator position at ACMC Healthcare System?
The Patient Centered Care Coordinator (PCCC) will work collaboratively with the multidisciplinary care team to coordinate and facilitate services around the patient and family's needs. Position will use assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination. Conducts outreach to a defined population to ensure timely and efficient care delivery across the continuum of care. Utilizes technological tools to manage populations.
Graduate of an Accredited School of Nursing; BSN preferred.
Current State Licensure as a Registered Nurse (RN), Specialty Certification in Case Management preferred.
Three to five years of nursing experience required in the Acute Hospital setting and or Primary Care Provider Office. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
