Key Responsibilities
- Program Oversight – Oversee the programmatic development, planning, and management of Case Management, Social Work, Utilization Management, and post-acute services coordination.
- Patient Care Facilitation – Ensure efficient patient care facilitation and seamless movement through the hospital continuum.
- Program Development – Develop and implement hospital case management programs to improve patient outcomes and care progression.
- Team Leadership – Lead a team of case managers and social workers, providing guidance and support.
- Collaboration – Collaborate with physicians, nurses, and healthcare teams to optimize patient care plans.
- Compliance & Evaluation – Monitor and evaluate care coordination strategies, ensuring compliance with healthcare regulations.
- Strategic Initiatives – Work closely with leadership to enhance hospital-wide care management initiatives.
Qualifications
Required
- Current state RN License (Montana or compact state).
- Five years of case management experience in a healthcare setting.
- Excellent communication and customer relations skills with a patient-centered approach.
Preferred
- Master’s degree in healthcare administration, nursing, or a related field.
- Three to five years of supervisory experience in case management.
- Formal case management training or certification.
