Classification : Non-Exempt. Thirty-five-hour (35.00) week; evenings and weekends expected for meetings, workshops, and seminars.
Position Summary : The Housing Case Manager actively assists individuals experiencing housing instability or homelessness by conducting comprehensive needs assessments, developing personalized case plans, and connecting them with appropriate housing resources, support services, and community agencies to secure and maintain stable housing while addressing underlying challenges that may contribute to housing insecurity.

Qualifications :
Two-year degree in health / human services or another related field; OR
Three years' experience working in a nonprofit social service agency providing direct client related services to diverse populations (e.g., persons of color, LGBTQI community members, people who use drugs, individuals with SMI diagnoses, people experiencing homelessness, chronic illness management, etc.). Familiarity with and experience working in housing or working with individuals at risk of or experiencing homelessness preferred.
Must have access to a reliable and insured vehicle that meets DMV standards, along with a valid NYS driver's license, for necessary travel.
Computer skills and proficiency in Microsoft Office and Outlook is a must; Electronic Health Records systems preferred.
Ability to work with diverse colleagues and clients, in a respectful and learning manner.
Effective communication and documentation skills, including familiarity with case note requirements for client chart completion, assessments, and service planning.
Experience delivering stigma-free, non-judgmental, and culturally competent services.

Primary Responsibilities :
Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F HIV Confidentiality Law, HIPAA Federal Privacy Rules, and 42 CFR Part 2 regulations for protected health information related to substance use.
Observe ACR Health policies and procedures, and other relevant regulations for client and agency data security. Protect agency data in accordance with confidentiality requirements.
Initiate and complete the process to secure decent, affordable, permanent housing.
Provide long term housing rental assistance and housing retention services to enrolled clients in maintaining stable housing to ensure long term housing security.
Complete program enrollments, intake / screening processes, review of program applications, and confirm client eligibility documentation for all enrolled individuals.
Conduct assessment and eligibility determination activities, as well as annual home inspections, and income / additional eligibility verifications for all enrolled clients.
Provides care coordination and necessary referrals to addressing all enrolled client's overall health and housing needs.
Assist with completing paperwork associated with securing recertification for housing related benefits, income maintenance, and essential housing needs.
Assist clients to secure services that maximize their tenure in housing; increase their ability to maintain their household and finances, independently perform activities of daily living, uphold the terms of their lease; prevent lease violations; intervene and mitigate crisis situations; maximize tenant safety and security; and prevent avoidable evictions.
Communicate, engage, advocate on behalf of tenants and mediate conflicts with landlords.
Provide case management services to increase consumer's capacity to live independently and self-sufficiently.
Complete and regularly updates Housing Assessment and individual service plans in conjunction with the client, Care Manager, when one is assigned, and other related service providers.
Conduct multidisciplinary case conferences and coordinate services and referrals with services and clinical providers that facilitate a client's engagement and retention in care.
Maintain complete and accurate statistical information; prepare all required monthly and program reports as assigned; ensure thorough, up to date and complete client records.
Complete / submit all required documentation within designated time frames.
Participate in agency case conferences as scheduled.
Maintain working knowledge of HOPWA program regulations, policies, and procedures.
Attend all required and recommended staff meetings, trainings, and in-services.
Perform other duties as assigned by Housing Program Manager, Director of Family, Education & Support Services, Chief Program Officer and / or Chief Executive Officer.
