Permanent (Full Time)
Pay Rate: $240,000-$260,000/Yearly.

Summary:
- Primarily responsible to assess, evaluate and treat patients in the hospital/medical office setting, under the supervision and collaboration of a physician, by using advanced knowledge and skill to identify abnormal conditions, diagnose health problems, develop and implement treatment plans, evaluate patient outcomes, and collaborate with or refer to a physician in the hospital/medical practice for further care management.
- Will see patients of a variety of ages: Pediatric through Geriatric
- Basic laboratory services are available and basic procedures may be performed.
- Opportunities for teaching exist with Health System Family Medicine residents.
- Must adhere to the Ethical and Religious Directives for Catholic Health Care Services.
- May be required to maintain active membership status on the medical staff of SJHS-SB and/or SJHS-Ply.
- Actively demonstrates the organization’s mission and core values and conducts oneself at all times in a manner consistent with these values.

Submission Requirements
- TRULY Board Eligible (within 5 years of residency) or Board Certified in Family Medicine (preferred) or Internal Medicine Required
- IN license (or willing to obtain) Required
- DEA (or willing to obtain) Required
- COVID-vaccination Required
- Provider will be working with all ages - must have experience working with pediatrics-geriatrics Required
- EPIC EMR knowledge Highly Preferred
