We are seeking a compassionate and experienced Care Coordinator to join our healthcare team. The ideal candidate will have 3-5 years of experience in a clinical setting and a strong commitment to providing high-quality patient care. This role involves supporting healthcare providers, managing patient records, and ensuring efficient clinical operations.
Key Responsibilities :
- Patient Care : Assist healthcare providers with patient examinations, treatments, and procedures. Take and record vital signs, medical histories, and patient information.
- Clinical Support : Prepare examination rooms, sterilize instruments, and ensure all necessary supplies are available.
- Patient Communication : Educate patients about treatment plans, medications, and follow-up care. Provide emotional support and address patient concerns.
- Administrative Duties : Schedule appointments, manage patient records, and handle billing and insurance documentation. Ensure accurate and timely entry of patient information into electronic health records (EHR) systems.
- Compliance and Safety : Adhere to all healthcare regulations and safety protocols. Maintain a clean and safe working environment.
- Documentation : Maintain accurate and up-to-date patient records, including care plans, progress notes, and any changes in patient status.
- Crisis Intervention : Respond to patient crises and emergencies, coordinating necessary interventions and support services.
- Quality Improvement : Participate in quality improvement initiatives to enhance patient care and outcomes.
