Job Description:
The Nurse Practitioner (NP) is a key team member who is part of the clinical operations team and will provide assessments to patients in our clinic in the New Orleans area.
Responsibilities:
The responsibilities will include clinical assessments, health education, physician referrals, care planning, coordination of care activities, case management referrals with clear documentation according to AOH guidelines and protocols. The NP is expected to meet AOH standards of care for quality, service, productivity and assesses patient health by interviewing patients in our clinic or by using our telemedicine platform.
- Perform visits for patients for Annual Wellness Visits, Advanced Care Planning assessments, health risk screenings, transitions of care, and physical assessments.
- Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
- Maintains professional appearance while conducting in-home or telemedicine visits.
- Works collaboratively with physicians, social workers, family, key care givers and any appropriate ancillary medical personnel.
- Is compliant with HIPAA, OSHA, Quality, CLIA, State Board of Nursing and Federal DEA Board of regulations.
- Participates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns and adheres to all applicable laws, regulations and ACT One Health policies and procedures.
- Performs other duties as assigned.
Qualifications:
- Demonstrated record of consistently achieving clinical performance metrics
- Proven ability to perform comprehensive, adult/geriatric physical exam
- Demonstrated ability to identify, diagnose and document chronic disease in adults
- Demonstrated ability to recognize conditions, characteristics, and behaviors
- Able to independently perform comprehensive physical exam
- Proven ability to pursue feedback, continuing education, and professional development
Requirements:
- Current RN and NP licensure in state of practice, Board certification by AANP or ANCC and Current CPR certification
- Minimum 3- year clinical experience as a Nurse Practitioner in geriatric, adult, emergency, or family practice setting
- Must have a valid state driver license
- Must have proof of adequate automobile insurance coverage for the state of residence.
- Must be able to travel by public transportation, and airplanes
- Must be able to independently travel to assigned locations up to a 50 miles radius from their home zip code
- Experience in managed care case management and/or care coordination a plus
- Home health experience preferred
- Monday - Friday

Salary : $116,578 - $126,825
