š Job Description:
As aĀ Clinical Assessment Manager ā Care Delivery, you'll play a crucial role in the clinical assessment process, including onboarding, reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation. You'll ensure compliance with Department of Health policies and serve as a valuable resource to our Intake, Enrollment, and Care Delivery teams.
š Key Responsibilities:
- CollaborateĀ with intake, enrollment, and care delivery teams for in-home or virtual assessment appointments.
- ConductĀ in-home assessments using the UAS-NY to determine eligibility for Community Based Long Term Care (CBLTC) Services.
- EvaluateĀ and review previous assessments to ensure accurate documentation in the EMR.
- PromoteĀ the PACE program through presentations, lectures, and meetings.
- EducateĀ potential and new PACE participants on plan benefits and services.
- EscalateĀ questions or concerns to the quality review team for appropriate handling.
- CompleteĀ and/or reassess the PCAT Tasking Tool for home assistance needs.
š¼ Essential Skills:
- Active and unrestricted NYS Registered Nurse license.
- Minimum of one year of experience with a frail or elderly population.
- 3-5 years of experience in MLTC, LTHHCP, MAP, PACE, or other managed care programs.
- 5 years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning.
- Proficiency in UAS-NY, Lenavi, CRM platforms, and Microsoft Office.
- Excellent communication and interpersonal skills.

- Highly motivated and self-driven.
- Positive attitude with the ability to multi-task in a constantly changing environment.
- Flexibility to meet deadlines, including occasional weekend work.
- Customer service-oriented with strong clinical management skills.
š¢ Work Environment:
- Professional office environment with standard office equipment.
- Schedule: 8:30 AM ā 5:30 PM, 40 hours per week.
- Involves in-home or virtual assessments; some travel required.
- Must be medically cleared for communicable diseases and have up-to-date immunizations.
Pay and Benefits
The pay range for this position is $110000.00 - $115000.00/yr.

This is a fully onsite position in Flushing,NY.
