Intake Nurse

Job Description

Seeking a dedicated Intake Nurse to join our large hospice organization. Responsibilities include coordinating referrals, verifying and entering physician's orders, determining the appropriate level of care for patients, reviewing documentation for facility placement, and coordinating orders with pharmacists. The role demands close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and their families.

• No patient hands on patient care, this is an office position
• M-F schedule between hours of 8am and 7pm, will rotate

Hard Skills

• LPN or RN license in the State of Utah
• 1 year experience in a skilled nursing facility
• BLS certification

Responsibilities

• Coordinate referrals
• Verify and enter physician's orders
• Identify the level of care required by patients
• Review documentation to determine facility placement
• Coordinate orders with pharmacists
• Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families

Additional Skills & Qualifications

• Experience with MatrixCare or Point of Care EMR

The pay range for this position is $32.00 - $32.00
