Job Title: Care Navigator

Job Description

The Specialty Authorization Team is looking for a Care Navigator to process authorizations and follow-ups for DME, MHM, DAHS, and reductions/denials. This role involves supporting vulnerable populations with chronic or complex conditions.

Responsibilities

• Process authorizations and follow-ups for DME, MHM, DAHS, and reductions/denials.
• Make outbound calls related to healthcare services.
• Coordinate care and manage cases for individuals with disabilities and/or vulnerable populations.
• Support insurance-related inquiries and processes.

Essential Skills

• 2 years of experience in a managed care environment or working with people who have disabilities and/or vulnerable populations with chronic or complex conditions.
• Experience in healthcare, outbound calls, and insurance support.
• LPN, LVN, or similar license required.

Additional Skills & Qualifications

• LPN, LVN, or similar license required.
• Strong skills in care coordination and case management.

Work Environment

This position is fully remote, requiring a strong internet connection. The work hours are Monday through Friday from 8:00am to 5:00pm, and the dress code is business casual.

Pay and Benefits

The pay range for this position is $18.00 - $21.00
