Job Title: Patient Navigator
Job Description
We are seeking a dedicated and compassionate Patient Navigator to assist with daily reception activities and ensure all processes needed for patient services are initiated. The ideal candidate will identify and assist individuals in accessing needed preventive and primary health services, with an emphasis on linking patients to a medical home. You will serve as an advocate for clients, providing assistance and referrals to obtain essential support services, and establish relationships with community health and human services providers.
Responsibilities
- Assist with daily reception activities and ensure all processes needed for patient services are initiated.
- Identify and assist individuals in accessing needed preventive and primary health services.
- Provide assistance and referrals to obtain essential support services.
- Establish relationships with community health and human services providers.
- Communicate effectively with team members and patients regarding all aspects of service.
- Handle telephone and mail contact with patients.
- Schedule appointments, collect fees, and explain services and charges for patient services.
- Distribute materials to patients, including patient satisfaction cards, patient rights, and other appropriate materials.
- Verify all insurance and set fees based on the sliding scale fee.
- Facilitate and monitor the scheduling and arranging of medical and dental appointments for patients and their families.
- Provide follow-up services for newly diagnosed individuals who require additional services.
- Provide basic health information and education.
- Track referrals to ensure patient access and support patients in overcoming barriers to care.
Essential Skills
- Experience with patient vitals and injections.
- Proficiency with electronic medical records (EMR) systems.
- Prior medical office administrative experience.
- Strong patient care and nursing clinical skills.
- Data entry proficiency.
- Excellent customer service skills.
Additional Skills & Qualifications
- Friendly personality.
- Knowledge of medical office software.
- Bilingual in Spanish or Creole is an asset.
Work Environment
You will work in a high-volume, fast-paced patient environment. The role requires an upbeat attitude and strong customer service skills. You will be part of a community health center that provides comprehensive primary, preventive, and behavioral health services to all individuals, regardless of insurance status or ability to pay, especially the underserved and vulnerable.
Pay and Benefits
The pay range for this position is $20.00 - $22.00/hr.
