Pharmacist

Job Description

Provide pharmaceutical services to long-term care and institutionalized patients, ensuring quality and patient safety. Participate in and oversee the dispensing of medication in compliance with regulations.

Responsibilities

• Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
• Make recommendations for therapy changes to physicians and facility staff as needed.
• Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
• Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with healthcare team members.
• Provide clinical and technical support to the healthcare team.
• Document all communication with physicians, nurses, other healthcare team members, patients/residents, and their family members in the medical record.
• Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as their disposal.
• Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and company policy.
• Inform the Pharmacy Manager of medication-related errors.
• Assist facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
• Perform other tasks as assigned.

Essential Skills

• Bachelor’s Degree in Pharmacy required; Pharm D preferred.
• One year of experience in long-term care preferred.
• PA Pharmacy License required.
• Knowledge of pharmacy laws and federal and state regulations concerning long-term care and pharmaceutical care practices.
• Ability to work cooperatively as a member of a team.

Work Environment

Hours are 10:00 AM to 11:30 PM, following a 7/7 rotation. When a Saturday and Sunday fall within this rotation, the hours will be 10:00 AM to 8:30 PM for the weekend and 1:00 PM to 11:30 PM on weekdays.

Pay and Benefits

The pay range for this position is $60.00 - $60.00/hr.
