In this position, you’ll ensure the highest standards of care by overseeing quality assurance of clinical documentation, creating plans of care, managing patient cases, and providing general clinical support across all disciplines. Key Responsibilities
- Quality Assurance (QA) of Documentation: Review and ensure clinical documentation (e.g., OASIS assessments, face-to-face encounters) meets CMS and Florida AHCA standards for compliance and reimbursement, primarily through remote review with occasional on-site collaboration.
- Create Plans of Care (POCs): Develop and update individualized POCs in collaboration with physicians and our multidisciplinary team, ensuring they address the unique needs of geriatric patients.
- Case Management: Manage patient cases, coordinating with referral sources, nurses, therapists, social workers, and families to ensure timely and effective care delivery while maintaining clear communication across all parties, using both remote tools and on-site visits as needed.
- General Clinical Support: Provide guidance to our diverse clinical staff, including nurses, therapists, and home health aides, assist with OASIS assessments, and support staff training on clinical protocols to enhance care quality, with a mix of remote and on-site support.
- Timeliness and Communication: Uphold Actify Home Care’s commitment to punctuality by ensuring care is delivered on schedule, and foster seamless communication with referral coordinators, patients, and staff to keep everyone informed and aligned, leveraging hybrid workflows.
- Licensure: Active RN license in Florida.
- Experience: Minimum of 1 year of clinical experience in home health or geriatric care, with a strong understanding of CMS Conditions of Participation.
- Skills:
- Proficiency in OASIS assessments and CMS documentation requirements.
- Excellent organizational skills to manage cases and ensure timely care delivery in a part-time hybrid setting with flexible hours.
- Superior communication skills to coordinate with referral sources, staff, and families, both remotely and on-site.
- Comfort with remote work tools (e.g., office products, electronic health records) for hybrid responsibilities.
- Preferred:
- Experience with QA processes and creating POCs for geriatric patients.
- Proficiency with Kinnser/WellSky software for documentation, care planning, and case management.
- Part-time flexible schedule with flexible hours to suit your availability.
