JOB SUMMARY :
The home care aide plays a vital role in providing compassionate and reliable services to clients in their homes.
PRIMARY RESPONSIBILITIES :
- Deliver direct patient care based on individual client needs.
- Assist with personal hygiene, including bathing, grooming, and mouth care.
- Support mobility through transfers, repositioning, and ambulation.
- Implement basic nursing care and prescribed exercises under the guidance of healthcare professionals.
- Prepare meals catered to clients' preferences and dietary requirements.
- Engage clients with companionship and activities tailored to their interests.
- Perform light housekeeping tasks as required.
- Collaborate effectively with team members to ensure high standards of healthcare.
- Maintain accurate and complete records as per agency requirements.
- Report any changes in clients' physical or emotional conditions promptly.
- Utilize the EVV app and / or time sheets to document tasks correctly.
- Demonstrate reliability by consistently arriving on time for shifts.
- Respect and adapt to the values of clients and their families.
- Encourage clients' independence while respecting their capabilities.
- Complete all duties outlined in the care plan.
- Communicate any questions or concerns to Action Health Staffing immediately.
- Prioritize patient confidentiality and uphold the patients' Bill of Rights.
- Report any suspected abuse, neglect, or unsafe living conditions without delay.
- Always adhere to the Plan of Care and refrain from administering medications.
EQUIPMENT USED : Personal transportation is required, along with medical equipment such as a blood pressure cuff and stethoscope as per care plans.
This position involves a significant amount of direct patient interaction within diverse home environments. Physical requirements include the ability to lift approximately 50 lbs. The Certified Nursing Assistant must be adaptable and willing to accept assignments based on availability and client needs, with no guaranteed hours or assignment length. Travel is necessary within a specified area.
