What are the responsibilities and job description for the RN position at Action Home Care?
Position Defined:
Treat patients, educate patients and the public about various medical conditions, provide advice and emotional support to patients’ family members, record patients’ medical histories and symptoms, perform diagnostic tests and analyze results, administer treatment and medications, and help with patient follow-up and rehabilitation.
Essential Duties:
- Initial and ongoing comprehensive assessment of the client’s needs, including OASIS assessments at appropriate time points.
- Initiating the plan of care and revising as necessary.
- Providing those services and/or treatments requiring substantial and specialized nursing skill
- Counseling the client and family in meeting nursing and related needs.
- Initiating appropriate preventive and rehabilitative nursing procedures.
- Preparing clinical and progress notes.
- Coordination of services.
- Referral to other services as needed.
- Informing the physician and other staff of changes in the client’s needs
- Evaluating outcomes of care
- Supervising LPN/LVNs, HHAs/CNAs and paraprofessionals providing services
- Assigning home health aide to a specific patient.
- Participating in in-service programs
- Planning for the discharge from service.
Minimum Requirements:
- Graduation from an accredited nursing program and current Florida licensure.
- Skill in the use of computers, preferably in a PC, Windows based operating system.
- Familiar with basic CPR skills
- Excellent interpersonal, oral/written communication, detail oriented skills
- Strong organizational skills
- Ability to manage numerous projects, tasks and priorities to achieve desired goals.
- Maintain confidentiality and integrity of position.
Work Environment and Physical Demands:
The work environment may involve hazards, risks or discomforts typical with patient care. When the employee is confronted with a variety of duties susceptible to different methods of solution which, in turn, places a correspondingly higher demand upon resourcefulness and concentration. The work requires considerable physical exertion.
