At Activate Care, we’re on a mission to improve health equity and drive improved health outcomes across the country. Our Community Care Record platform, Care Link, enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs (HRSN). Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Our goal is simple: address individuals’ unmet HRSNs, increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend.
Activate Care has partnered with Commonwealth Care Alliance & Commonwealth of Massachusetts to provide supportive temporary emergency shelter to families under the Executive Office of Housing and Livable Communities’ Emergency Assistance program to assist a rapidly growing number of unhoused families, including thousands of families migrating from Haiti due to natural disasters and civil unrest.
Role Overview:
The Housing Stabilization Manager leads a team that conducts housing stabilization activities to ensure tenancy preservation for families who have transitioned out of CCA-supported family shelter sites into permanent housing across Massachusetts. This includes monthly check-ins with families and landlords and intervention in cases where tenancy is at risk.
Responsibilities:
- Supervise a team of housing stabilization specialists (HSS) across family shelter sites.
- Create and deliver training on housing stabilization resources and approaches.
- Lead performance management program for HSS efforts across shelter sites.
- Implement best practices for accurate, timely data and drive initiatives to ensure continuous performance improvement.
- Regularly review and present programmatic data related to housing stabilization to ensure that all family shelter leadership are aware of monthly goals and performance.
- Proactively work with site leadership to ensure compliance with program requirements and accuracy of data input.
- Provide expert consultation to HSS on complex tenancy preservation cases to ensure successful resolution.
- Other duties as assigned.
Qualifications & Skills:
- Bachelor’s Degree required, ideally in human services, social work, public health or related industry
- Valid driver’s license, reliable transportation, and ability to travel throughout Massachusetts
- 3 years providing case management services for populations experiencing homelessness
- 2 years with directly supervising other staff
- Creatively leverage resources to support families through case management specific to populations experiencing homelessness
- Demonstrated ability to supervise and lead a team and deliver effective training
- Previous experience in shelters, social services, or related environments to support with housing navigation
- Excellent interpersonal and communication skills with the ability to work with people of various skill and experience levels
- Proactively identifies problems and areas of improvement, providing solutions
- Collaborate and create an environment of mutual trust and respect
- Exceptional organization and self-motivation to work independently and collaboratively as needed
- Advanced Microsoft Office computer skills
- Ability to multi-task and switch gears quickly / effectively
- Cultural humility: ability to communicate effectively with people from various backgrounds and work respectfully across demographic, socioeconomic, language, and all other constituents that represent diverse cultures of communities
Nice to Have’s:
- Experience working in family shelters and/or working with individuals that have recently migrated to the US, particularly from Haiti
- Bilingual (Haitian Creole or Spanish)
