Job Summary:
The Home Health Office LPN plays a critical role in ensuring the smooth operation of clinical and administrative functions within a home health agency. This position serves as a liaison between field staff, physicians, laboratories, and insurance providers while also supporting quality assurance and performance improvement initiatives. The ideal candidate is highly organized, detail-oriented, and skilled in effective communication to enhance patient care coordination and regulatory compliance.
Key Responsibilities:1. Communication & Coordination
- Act as a primary point of contact between the home health agency, physicians, laboratories, and field clinicians (RNs, PTs, OTs, HHAs).
- Facilitate timely and accurate transmission of physician orders, medication changes, and treatment plans.
- Ensure lab results, diagnostic reports, and physician recommendations are received, reviewed, and communicated to the appropriate staff.
- Coordinate patient referrals and schedule necessary follow-ups based on physician recommendations.
2. Insurance Authorization & Compliance
- Obtain and verify insurance authorizations for home health services, ensuring timely renewals to avoid service interruptions.
- Communicate with payers regarding coverage, approvals, and denials while documenting all interactions appropriately.
- Ensure documentation meets insurance and regulatory requirements to support claims processing and reimbursement.
3. Quality Assurance & Performance Improvement (QAPI)
- Assist in the development, implementation, and monitoring of Quality Assurance and Performance Improvement (QAPI) initiatives.
- Participate in internal audits, reviewing clinical documentation to ensure accuracy, completeness, and compliance with Medicare/Medicaid and accrediting body standards.
- Assist in tracking and analyzing performance metrics, identifying trends, and implementing corrective actions as needed.
- Educate field staff on best practices, policies, and procedures related to quality care and compliance.
4. Administrative Support & Clinical Oversight
- Maintain accurate patient records in the electronic medical record (EMR) system, ensuring timely updates and compliance with HIPAA regulations.
- Support the onboarding and training of new field staff, reinforcing policies, procedures, and documentation standards.
- Assist with incident reporting, patient complaints, and resolution of service-related issues.
- Participate in case conferences and interdisciplinary team meetings to support care planning and coordination.
Qualifications & Requirements:
- Licensure: Active and unencumbered Licensed Practical Nurse (LPN) license in Louisiana.
- Experience: Minimum of 1-2 years in home health, healthcare administration, or a related field.
- Skills:
- Strong communication and interpersonal skills to effectively coordinate with physicians, labs, staff, and patients.
- Proficiency in EMR systems, Microsoft Office (Word, Excel, Outlook), and medical documentation.
- Knowledge of Medicare/Medicaid guidelines, home health regulations, and insurance authorization procedures.
- Detail-oriented with excellent organizational and multitasking abilities.
- Certifications: CPR certification (preferred).
Work Environment & Schedule:
- Office-based role with occasional field visits as needed.
- Monday–Friday schedule with flexibility based on agency needs.
