The Patient Assessment Coordinator is responsible for facilitating and communicating appropriate and timely admissions to the hospital. In addition, the PAIC coordinates the completion and transmission of the Inpatient Rehab Facility Patient Assessment Instrument (IRF-PAI) with interdisciplinary team members accurately and within appropriate time frames in accordance with Centers for Medicare and Medicaid Services.
Specifically, the PPS Coordinator oversees all aspects of data collection on the IRF-PAI, ensures the presence of supporting documentation, and provides staff education to ensure compliance with internal policy and external regulatory requirements, including applicable Joint Commission standards, and CMS guidelines. In addition, the PPS Coordinator develops and interprets policies and procedures and acts as the primary resource for immediate problem solving in regards to IRF-PAI scoring. This position requires a positive working relationship with interdisciplinary teams.

MINIMUM QUALIFICATIONS :
- Be a graduate of an accredited Registered Nurse (RN), Occupational Therapy (OT), Physical Therapy (PT) or Speech Language Pathology (SLP) program with current Texas licensure
- Two years clinical experience in the care of patients in an acute hospital, rehabilitation or LTACH setting required
- Experience in IRF PAI or MOS coordination (preferred)
- Must have leadership ability and good communication and interpersonal skills
- Working knowledge of Medicare guidelines for Rehabilitation (acute rehab criteria, level of care requirements and medical necessity)
- Ability to function within an interdisciplinary environment
- Ability to read and speak English clearly and be understood
- Understand Medical terminology, regulations and parameters of third party reimbursements, and coding guidelines
- Have basic computer knowledge including Outlook, Excel, Word, PowerPoint, and data management software required
ESSENTIAL DUTIES :
Coordinates the process of providing accurate and timely data to the appropriate entities involved in the Prospective Payment System (PPS) and quality reporting program for Medicare patients.
Oversees all aspects of data collection on the IRF-PAI, ensures the presence of supporting documentation, and provides staff education to ensure compliance with CMS guidelines.
Collaborates with Physical Medicine & Rehabilitation physicians to ensure timeliness and accuracy of required documentation.
Responsible for monitoring patient charges for correct coding and compliance with CMS guidelines.
Assumes the task of reviewing each IRF-PAI form completely, accurately and timely
Assists with the collection of data and interpretation of performance improvement outcome information. Perform regular chart audits to ensure quality documentation.
Educates staff on Section GG items and monitors the accuracy in the scoring of the functional status and reflection of the patient’s needs
Sets completion dates for the multidisciplinary team
Develops and interprets policies and procedures and acts as the primary resource for immediate problem solving in regard to Medicare's IRF prospective payment system (GG scoring, impairment group coding, tier distribution), coverage and classification criteria, and quality reporting program requirements
Maintains regulatory requirements, including all state, federal, CIHQ and Hospital Compliance Plan
Works collaboratively with the team to optimize patient outcomes and maximize reimbursement for the organization.
Educates staff regarding the application of the Section GG items, it’s monitoring and implementation.
Performs regular chart audits to ensure quality documentation. Communicate with team members and educate team members in any area of the CMS guidelines that require such.
Assists with assigned administrative duties, i.e. patient rounding
