Responsibilities
- Client Assessment and Support: Conduct thorough assessments of client needs, focusing on behavioral health, food insecurity, transportation, housing, and other essential community services.
- Knowledge Development: Gain in-depth knowledge of available community support services and programs, including the referral process for each.
- Veteran-Specific Expertise: Become adept in military benefits, documentation, and health insurance navigation to support our veteran community effectively.
- Referral Management: Provide well-coordinated referrals, both internally and externally, ensuring a seamless transition for clients.
- Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access, utilizing local resources and supports to optimize care.
- Educational Support: Offer education and resources to clients as needed, enhancing their understanding and engagement in their own care.
- Documentation and Data Management: Meticulously document all client interactions and data, participating in data analysis and reporting.
- Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided.
- Collaboration and Outreach: Work closely with clinical and program staff, maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners.
- Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences.
- Follow-Up: Ensure continuous care by providing consistent follow-ups with patients, clients, and partners.
- Teamwork: Collaborate with various community service providers, including clinical, nutrition, and ancillary support services.
- Technology Utilization: Employ virtual/tele-health and texting program equipment for remote client interactions.
- Additional Responsibilities: Be adaptable to perform other duties as assigned.

Qualifications For a Care Coordinator Specialist
- Accredited degree in Social Work, Public Health or related field.
- Experience in case management/care planning, customer service, and enrollment process for medical insurance products, preferred.
- Must be able to comfortably work with Microsoft office products.
- Must have excellent communication skills. Some regional travel required.
