CARE COORDINATOR SPECIALIST

Adagio Health Inc is looking to hire one (1) full-time Care Coordinator Specialist for its Erie office.

We are looking for compassionate and driven individuals to join our team as a Care Coordinator Specialist. This role is integral to our mission of providing comprehensive support and care to a diverse range of clients, including those with specific needs such as HIV, veterans, and others requiring specialized services.

Responsibilities

  • Client Assessment and Support: Conduct thorough assessments of client needs, focusing on behavioral health, food insecurity, transportation, housing, and other essential community services.
  • Knowledge Development: Gain in-depth knowledge of available community support services and programs, including the referral process for each.
  • Veteran-Specific Expertise: Become adept in military benefits, documentation, and health insurance navigation to support our veteran community effectively.
  • Referral Management: Provide well-coordinated referrals, both internally and externally, ensuring a seamless transition for clients.
  • Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access, utilizing local resources and supports to optimize care.
  • Educational Support: Offer education and resources to clients as needed, enhancing their understanding and engagement in their own care.
  • Documentation and Data Management: Meticulously document all client interactions and data, participating in data analysis and reporting.
  • Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided.
  • Collaboration and Outreach: Work closely with clinical and program staff, maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners.
  • Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences.
  • Follow-Up: Ensure continuous care by providing consistent follow-ups with patients, clients, and partners.
  • Teamwork: Collaborate with various community service providers, including clinical, nutrition, and ancillary support services.
  • Technology Utilization: Employ virtual/tele-health and texting program equipment for remote client interactions.
  • Additional Responsibilities: Be adaptable to perform other duties as assigned.

  • Navy: Hospital Corpsman (HM), Social Work Officer (SWO), Personnel Specialist (PS)
  • Air Force: Aerospace Medical Service Technician (4N0X1), Mental Health Technician (4C0X1), Personnel Specialist (3S0X1)
  • Coast Guard: Health Services Technician (HS), Yeoman (YN), Marine Science Technician (MST) (with relevant experience)
  • Space Force: Medical Support, Personnel Administration, Behavioral Health (prior service from other branches)

Qualifications For a Care Coordinator Specialist

  • Accredited degree in Social Work, Public Health or related field.
  • Experience in case management/care planning, customer service, and enrollment process for medical insurance products, preferred.
  • Must be able to comfortably work with Microsoft office products.
  • Must have excellent communication skills. Some regional travel required.
