Overview
Ensure that the facility is compliant with the guidelines for the Minimum Data Set (MDS)
Medicaid and Medicare RUGs/PDPM
Medicaid Case Mix
Medicare Criteria including the supportive documentation requirements.
Knowledge of case management with Managed Medicare plans. Understanding of how MDS accuracy affects reimbursement and regulatory compliance. Including knowledge of Quality Measures and Five Star ratings criteria. The successful candidate will possess, at minimum , the following qualifications: Licensed Nurse in the State of Ohio or eligible for licensure in Ohio
Experience in MDS.
Duties
- Assess patient needs and develop individualized care plans that align with their medical and personal goals.
- Coordinate and facilitate communication between patients, families, and healthcare providers to ensure comprehensive care.
- Manage health information effectively, utilizing systems such as Epic for documentation and tracking patient progress.
- Conduct thorough reviews of medical records, including knowledge of ICD-10 coding and anatomy to ensure accurate assessments.
- Oversee discharge planning for patients transitioning from inpatient care to home or other facilities, including hospice care when necessary.
- Collaborate with primary care providers to ensure continuity of care and address any ongoing health concerns.
- Monitor patient progress and adjust care plans as needed based on evolving health conditions or feedback from the healthcare team.
Qualifications
- Strong knowledge in health information management and medical management practices.
- Familiarity with PCC software and other relevant healthcare systems.
- Understanding of ICD-10 coding, inpatient processes, discharge planning, hospice care, and primary care principles.
- Excellent communication skills with the ability to empathize with patients and their families.
- Proven ability to work effectively in a multidisciplinary team environment.
- Strong organizational skills with attention to detail in managing case documentation and follow-ups.
