The program, funded by the Family and Social Services Administration through the Division of Mental Health and Addictions, aims to deliver comprehensive case management services tailored for transition-aged youth (ages 14 to 26). The case manager will work closely with each youth, providing individualized treatment intensity to promote successful transitions into adulthood while supporting their mental and emotional well-being. This role offers the chance to implement creative treatment approaches and fosters community engagement alongside the youth.
This is a temporary position funded by a grant, set to conclude on June 30, 2025. Candidates should be aware that this role may be eliminated after this date if the grant is not extended.
Requirements :
40 hours per week - First Shift
Monday through Friday from 8 : 30 am - 5 : 00 pm
Bachelor's Degree in Social Work, Human Services, or Psychology
The position requires strong flexibility, effective time management skills, and excellent communication abilities.
Applicants should be able to follow directions and complete tasks independently while demonstrating a willingness to work with diverse groups, particularly those related to mental health and addictions.
