In this role, the RN will provide comprehensive care in a medical office environment, with a primary emphasis on phone nursing for the AMG - Woodcrest office. Key responsibilities include overseeing Chronic Care Management and Medicare Wellness programs across multiple locations, facilitating telehealth consultations with Nurse Practitioners at nursing homes, and managing nursing home triage and orders as necessary, all aimed at delivering high-quality patient care and support.
Eligible for medical coverage on your first day of employment, all other benefits will be effective the 1st of the month following hire date!
Requirements
40 hours per week - First Shift
Monday - Friday
7:30 am to 5:00 pm
Valid RN License in the State of Indiana
