As a Care Manager, you'll play a vital role in helping patients take control of their health and well-being. In this position, you’ll:
- Work closely with physicians and healthcare professionals in a cross-disciplinary, team-based setting.
- Work in conjunction with a psychiatrist and pediatric providers to bring the best level of care possible to their patients.
- Guide patients to valuable community resources and support systems.
- Motivate patients to achieve personal physical and mental health goals.
- Collaborate with physicians to create personalized treatment plans that meet the unique needs of each patient.

What You Bring:
