Responsibilities and Expectations
- You will reflect the company's mission and values in developing and maintaining professional working relationships with others.
- Perform initial and periodic assessments of clients to determine care needs, implement interventions, and evaluate the effectiveness of care to achieve positive outcomes.
The position collaborates with physicians/non-physician practitioners and the home health team to develop and update the client's home health plan of care at least every 60 days or more often as changes occur.
