**About the Role:**
We are seeking a skilled Physical Therapist to join our team at Adara Home Health. As a member of our team, you will have the opportunity to work with clients in their homes, providing personalized care and support.
The Physical Therapist will be responsible for administering physical therapy evaluations, participating in developing the Plan of Care, and providing physical therapy treatments, procedures, evaluations, and diagnostic tests for clients under physician direction.
Key Responsibilities:
- Translates exercises into functional activities or activities of daily living
- Provides instructions in the use and care of assistive devices
- Collaborates with other healthcare professionals to ensure comprehensive care
Requirements:
- Graduate of an accredited Physical Therapy program; currently licensed as a Physical Therapist in Minnesota
- At least two years of experience in a healthcare setting or equivalent experience
- Strong analytical and problem-solving skills
