">Schedule and Work Environment
This is a full-time position, Monday through Friday, with an on-call rotation. As a PT Case Manager, you will have the opportunity to manage your schedule during office hours and provide one-on-one client care that may take 1-3 hours per client.
You will be required to travel to client's homes in Cannon Falls, Red Wing, Lake City, and surrounding cities, which is why a reliable automobile and valid driver's license are essential for this position.
">Benefits and Compensation
We offer a competitive salary range of $80,000-$110,000 per year, plus productivity bonuses and benefits such as medical, dental, vision, life, AD&D, and pet insurance.
In addition to these benefits, you will also have the opportunity to participate in our 401(k) plan, which vests immediately.
">Required Skills and Qualifications
To be successful in this role, you will need to have at least two years of experience in a healthcare facility or home care field.
You must also have current licensure as a Physical Therapist (PT) in the State of Minnesota, proof of negative TB testing, and access to a dependable automobile.
">Responsibilities
Your primary responsibility will be to provide and oversee home health services for clients within the community, working collaboratively with the client, their family, and the healthcare team to develop and implement goal-centered care plans.
You will also be responsible for providing skilled care to clients and clinical oversight to professional and unlicensed personnel, building and maintaining respectful relationships with others.
">Working Conditions and Physical Demands
This position requires heavy lifting, transferring, and bending, as well as prolonged standing, sitting, and driving.
Must be adaptable to a variety of environments and community settings, including exposure to allergens and infectious diseases.
