Job Overview:
A Medical Records Clerk is responsible for managing and maintaining patient health records in healthcare settings such clinics and medical offices. They ensure that medical records are accurate, up-to-date, secure, and compliant with privacy laws and regulations. Medical Records clerk play a vital role in ensuring that patient data is organized, easily accessible, and protected.
Key Responsibilities:
- Medical Record Management:
- Organize and maintain patient health records, ensuring they are accurate, complete, and updated.
- Ensure that records are properly filed, either electronically (in an Electronic Health Record system, EHR) or in physical files, following organizational protocols.
- Assist in the retrieval and storage of medical records as needed by healthcare providers, ensuring easy access to critical patient information.
- Accurately enter patient information and medical data into electronic systems.
- Update patient records with new medical history, diagnostic tests, treatments, and procedures performed.
- Ensure patient demographic data (name, address, insurance details) and medical history are entered correctly.
- Ensure compliance with HIPAA (Health Insurance Portability and Accountability Act) and other healthcare privacy regulations to safeguard patient confidentiality.
- Ensure that medical records are kept secure and that access to records is restricted to authorized personnel only.
- Verify that records are compliant with regulatory standards and guidelines set by healthcare authorities or insurance providers.
- Respond to requests for medical records from patients, healthcare providers, or insurance companies in a timely and accurate manner.
- Ensure that all medical records released are in accordance with legal and ethical guidelines.
- Handle records requests for purposes such as billing, insurance claims, or continuity of care.
Qualifications:
- Education:
- High school diploma or equivalent is required.
- Post-secondary education or certification in Health Information Management, Medical Records Technology, or a related field is preferred.
- Experience:
- Experience working in medical records or health information management is beneficial but not always required.
- Familiarity with electronic health records (EHR) systems and medical coding/billing is highly valued.
