Position Summary:
Prosperity Point is seeking a Discharge Coordinator to support patients as they transition from residential treatment to post-discharge care. This role is critical in ensuring continuity of care by conducting initial patient contact, assisting in the development of recovery plans, maintaining accurate records in the Electronic Medical Record (EMR) system, and coordinating post-discharge services. The Discharge Coordinator collaborates with the clinical team to promote patient safety, adherence to treatment recommendations, and effective communication with referral sources.
Qualifications:
- High School Diploma or GED required.
- CPRS Certification Preferred (TN Only).
- CPR certification required within 30 days of employment.
- One year of experience in behavioral health or a related field preferred.
- 2 years of verifiable sobriety required if in recovery.
Essential Functions:
- Facilitate discharge planning and assist patients in creating recovery plans in the EMR.
- Conduct initial contact with patients within five days of entering residential services.
- Coordinate post-discharge arrangements, including scheduling follow-up appointments and assisting with service-related phone calls.
- Advocate for patients by addressing individual needs and coordinating with referral sources.
- Ensure patient safety and compliance with Bradford Health Services policies and state regulations.
- Maintain accurate and up-to-date documentation for patient records.
- Collaborate with clinical teams to align discharge plans with treatment recommendations.

- Regular attendance required, including participation in meetings and ongoing training.
- Perform other duties as assigned by the Clinical Manager.
