We are hiring a Targeted Case Manager for our growing team! The Targeted Case Manager will primarily be responsible for: (List high level duties)

Key Responsibilities:

• Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations.
• Charting for insurance purposes
• Maintain a positive, professional attitude toward residents, staff and volunteers.
• Create a safe and healthy environment for clients.
• Have a sincere commitment to client’s recovery.
• Transport clients to appointments when necessary.
• Monitor client's emotional well-being and behavior.
• Connect client with family and community resources.
• Connect client with legal entities when necessary.
• Motivate clients toward goal achievement.
• Meet with clients weekly.
• Help residents with long-term care plan development and educational resources.
  

• Bachelor’s degree in social work or human services field required.
• 1 year post grad work experienceor master’s degree required.
• Must have a current driver’s license.
• Basic computer skills.
• Ability to complete and submit reports.
• Be self-motivated.
• Have excellent documentation skills.
• Possess excellent written and verbal communication skills.
• Have a strong working knowledge of computer skills and use of search engines.
• Be able to handle crisis situations in a calm and supportive manner.
• Maintain a positive, professional attitude toward clients and staff.
