Integrated Care Coordinator for Mobile UnitIn collaboration with the Mobile Recovery Medical Care Coordinator and under the guidance of the Mobile Program Manager, the Integrated Care Coordinator will ensure the seamless coordination and delivery of essential services to clients. This role involves managing the care of specifically identified individuals, including tasks such as intake, discharge, billing, and service coordination.The Integrated Care Coordinator must recognize and respect each client's unique recovery journey, assisting in the development of a personalized care plan that aligns with their individual path. A solid understanding of the drug and alcohol treatment system, as well as a demonstrated commitment to the recovery community, is essential.This position serves as a point of contact for individuals recovering from substance use disorders, with the goal of preventing relapse and supporting long-term recovery. It combines direct client care with ongoing efforts to develop and enhance the integrated care model.ESSENTIAL FUNCTIONSCoordinate Holistic Care : Oversee every aspect of care for individuals on their recovery journey, ensuring they receive the support they need at every step.Empower Personal Discovery : Help individuals in recovery identify their passions, strengths, and goals, guiding them to unlock their potential and embrace areas for growth.Develop Tailored Recovery Plans : Partner with individuals to create personalized recovery plans, and ensure all aspects are well-documented for clear tracking and success.Foster Strong Community Connections : In conjunction with the Mobile Program Manager, cultivate relationships with local community partners, improving access to integrated care and ensuring no one falls through the cracks.Advocate for Recovery Initiatives : Serve as an advocate and ambassador for recovery-related initiatives, activities, and boards expanding pathways to recovery in rural communities.Guide the Transition to Self-Directed Recovery : Support individuals as they transition from professional-driven plans to self-guided recovery, empowering them for lasting success.Create Pathways to Resources : Actively seek and establish connections to valuable community resources that align with individual recovery goals, involving the full network of staff and stakeholders.Ensure Timely and Accurate Documentation : Maintain detailed, accurate records of all care provided, facilitating proper billing and financial processes.Identify and Overcome Barriers : Recognize internal and external challenges that may hinder access to community resources and create strategies to break down these barriers.Be Ready to Drive the Mobile Unit : Step in to drive the Mobile Unit, taking services directly to the people who need them most.Availability : Minimum of 40 hours per week, Monday- Friday, 8 a.m.- 5 p.m.BENEFITS : Medical Dental Vision 401k with 5% matchPTOQualifications, Education, and Experience : Minimum 1 year of experience working in social services Bachelor's degree in human services with minimum 1 year of experience (preferred) Certification as Associate Certified Coach (life coach certification), Community Health Worker Certification, CCAR (Connecticut Community for Addiction Recovery), Certified Recovery Coach (considered) Experience in substance abuse treatment (preferred). High school diploma or GED (required)REQUIRED TRAINING / CERTIFICATIONS : CPR / First Aid Certification required upon hire.Valid Michigan Chauffeur License required upon hire.MCBAP Certification or MCBAP Development Plan required upon hire.Recovery Coach Certification required or must be completed upon hire- if eligibleMust adhere to ATS training requirements.SKILLS NEEDED / EXPERIENCE : Strong written and verbal communication skills, with the ability to convey information clearly and effectively.Excellent decision-making and critical thinking abilities, with experience functioning within a multidisciplinary team. Demonstrates the capacity to think creatively and consider diverse perspectives.In-depth knowledge of recovery and addiction treatment models.Proficient in crisis intervention techniques, with the ability to de-escalate situations and problem-solve to ensure safety and stability.Demonstrated ability to maintain professional boundaries in relationships with staff, clients, and community members.This job description is intended to indicate the kinds of tasks and levels of work that will be required for this position.
