Description :
The Licensed Practical Nurse (LPN) at our withdrawal management facility provides essential medically monitored withdrawal management care and treatment to patients, ensuring continuity of care as part of a multi-disciplinary team. This role requires delivering care within a trauma-informed framework and upholding high standards of patient management. Activities and duties include, but are not limited to the following :
- Patient Care :
- Administer medications and treatments as prescribed, perform urine drug screens as needed.
- Perform nursing assessments and document patient health status.
- Assist with patient admissions, discharges, and transfers.
- Monitor and record vital signs and patient responses to treatments.
- Maintain detailed documentation of patient information, consultations, and treatment outcomes.
- Accurately record data in Medication Administration Records (MARS); review and transcribe new orders and provider instructions.
- Conduct regular chart audits and notify care providers of significant patient conditions, including pregnancies.
- Ensure proper storage and security of medications according to policy.
- Provide patient education and support on managing health conditions.
- Collaborate with RNs, physicians, and other healthcare professionals to plan and implement patient care.
- Ensure the cleanliness and safety of patient environments.
- Adhere to infection control protocols and facility policies.
