- Position reports to the Medical Director.
- Care for patients focusing on the continuity of care.
- Position is responsible for oversight to manage and track coordination of pro-active care for patients with chronic conditions as well as identify wellness and/or preventative service for clinical patients.
- Patient Centered Medical Home model.
- Experienced physician or recently completed resident will be considered.
- Doctor of Medicine or Doctor of Osteopathic Medicine
- Board Certified or Board Eligible Physician.
- Active, non-probationary physician license. Prescriptive authority.
- Proficiency in a language in addition to English, specifically Spanish is a plus
