The Licensed Independent Social Worker plays a vital role in working with patients to ensure there is both quality of care and appropriate access to care and resources as well as an environment that is safe for discharge. The social worker may initiate direct patient care services, family and patient counseling, discharge planning, crisis intervention, referrals as well as promotes educational programs focused on healthier behaviors. The LISW has access to confidential employer, patient, and financial information. Responsible for making independent decisions within scope of practice and in collaboration with team members. Provides direct patient care, to all ages through the continuum of care and often works with those who may be suffering with chronic, life threatening or altering diseases and disorders, helping connect patients with plans and resources in order to help them cope. Licensed Independent Social Workers can be required to aid with issues directly caused by trauma, disability, poor family circumstances, addictions and abuse.
Required Educational Degree: Master's Degree
Required Certifications, Credentials and Licenses: LISW-licensed independent social worker with current license in the state of Ohio with in 1 year of hire. CPR certification required; BLS - Basic Life Support
Required Experience: 1-2 years experience in social work. 3-5 years experience in direct patient care, utilization management, case management, acute care or discharge planning for Critical Access Hospital (CAH) positions
Preferred Experience: Previous experience in acute behavioral
