Position Summary:
The care coordinator is responsible for overseeing the disease management/case management/care coordination needs of attributed lives associated with the primary care centers. Under the direction of primary care providers, performs comprehensive assessment and care planning, education/coaching, and referrals to community resources and organizations. Care coordination and case management are crucial components of effective on-site care teams. The care coordinator, in collaboration with the healthcare team, coordinates the plan of care for the patient and family through the continuum of care, both for patients seen in the health centers as well as hospital discharges. This role is one of coordination of services, data collection, analysis, and monitoring of resource utilization with a systematic approach to interventions for improved patient outcomes, clinically, financially, and with patient satisfaction.
Care coordinator is trained in connectivity to health care coverage, community primary and preventative services, social support resources, and patient education related to these topics. They are experts on available programs and resources within their assigned designated service area and are aware of and sensitive to the unique needs of the patients, families, and community as a whole.
Educational Requirements/ Qualifications:
Must have either a current NYS RN license (PRI screen and certification preferred) or Master of Clinical Social Work certificate in good standing. BSN or CSW license preferred. Must have 5 years clinical experience. Case management/care coordination experience preferred. Requires excellent communication and collaboration skills. Requires ability to problem solve in complex situations. Must be able to work around interruptions and cope with stressful situations. Familiarity with MS Word, Excel; and internet use required; Familiarity with Meditech and eCW a plus, and will be required within 3 months of hire.

Salary Range:
$36.79 - $54.11 per hour
