RESPONSIBILITIES & DUTIES

  • Engage in direct support activities for clients of Adjoin as assigned at least 30% of total hours by:
  • Advancing client’s decision-making, productivity and participation in a range of daily activities. Facilitating the client’s inclusion in positive community experiences.
  • Promoting independence in daily activities while being mindful of client’s intellectual and physical disabilities and assisting with meals, cooking, household maintenance, grooming and dressing, as assigned. Maintaining current knowledge of client’s needs.
  • Implementing individual training and support programs under the direction of the case responsible person.
  • Maintaining positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community.
  • Participating in the evaluation of the client through completion of task analysis and other requested assessment devices.
  • Documenting required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required.
  • Providing positive advocacy when the individual’s preferences, needs, or talents are neglected or overlooked.
  • Addressing challenging behaviors proactively and respectfully, using methods outlined in plans and training.
  • Overseeing client activities and environment throughout shift to ensure the fulfillment of health and safety needs
  • Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.
  • Facilitate and coordinate the needs of the local office by:
  • Acting as coordinator for all office needs such as working with facilities managers, coordinating moves, communicating with alarm companies to resolve issues.
  • Greeting visitors and answering and directing phone calls in a professional and courteous manner.
  • Assisting with client tracking activities such as referrals and number of client hours as needed and directed.
  • Disseminating information to office personnel in unit offices as directed and coordinate local ‘all staff’ meetings.
  • Photocopying and maintaining inventory of forms, packets, supplies, including safety supplies such as PPE.
  • Managing and assisting with documentation by scanning, uploading, organizing and filing as needed.
  • Receiving, sorting and distributing incoming and outgoing faxes, mail, and packages appropriately.
  • Maintaining and manage petty cash in addition to tracking purchases and receipts.
  • Assisting with fleet coordination as needed.
  • Assisting with Pre-Hire and onboarding process for new hires as needed and as directed by management.
  • Ensuring location safety compliance by acting as local Safety Officer and as a member of the Adjoin Safety Committee including:
  • Plan, conduct, and ensure staff attendance at month safety meetings.
  • Participate in and apply information from quarterly organization-wide Safety Committee meetings.
  • Facilitate and coordinate Hep B vaccines and worker’s compensation administration, tracking and follow up.
  • Coordinate, participate in and follow up on both internal and external office inspections.
  • Plan, execute and track safety drills.
  • Maintain and update safety documents and required postings.
  • Track and facilitate required safety training and certification such as First Aid/CPR.
  • Manage and oversee all safety matters in the office, including fire extinguishers, first aid kits & safety supplies.
  • Complete safety orientation training for new hires.
  • Overseeing the maintenance of all office equipment, assuring office space is well maintained, orderly and a professional environment, maintaining inventory of office supplies, equipment and furniture.
  • Acting as the contact person for and gathering information for monthly newsletters.
  • Participating in Quality Assurance activities per company schedule as assigned.
  • Reserving venues for meetings and conferences; organizing social events as requested; making travel and room reservations as required.
  • Other duties as assigned

MINIMUM QUALIFICATIONS

  • 18 years of age or older.
  • High School Diploma or Equivalent.
  • Aptitude for confidentiality, organizational skills and good interpersonal skills.
  • Certification in an approved First-Aid and CPR class; or successful completion within 45 days of the date of hire.
  • Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.
  • Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.

POSITION REQUIREMENTS

  • Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
  • A Direct Support Facilitator works as scheduled by the Regional Manager and/or his/her designee. His/her schedule may be either part or full time, depending on the needs of the clients and Adjoin. Overtime may be required as dictated by operational needs, however, no holiday or overtime may be worked without the prior approval of the Direct Support Professional's supervisor.
  • Successful completion of pre-employment drug screen and background check.
  • Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
  • Acceptable DMV record.
