Telephonic RN Case Manager responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs with a focus on improving quality of life.
Coordinates, monitors and ensures that appropriate and timely primary, acute and long-term care services are provided to members across the continuum of care. Assumes a leadership role within the Interdisciplinary Care Team (ICT). Works directly with the member in the field (inpatient bedside, member's home, provider's office, hospitals, etc.) while collaborating with management.
The ability to multi-task, work independently, be creative and implement process improvements is necessary. Excellent written and verbal communication skills are needed in order to present information to staff, members, and providers. Knowledge of healthcare delivery along with community, state, and federal laws and resources is a plus.
1 year of experience in current case management is preferred along with the care of the elderly in a variety of geographic regions. Experience with in-home health, physician's office or public health is preferred.
Must be a licensed Registered Nurse in the state of Arizona or have a compact state license. A certified Case Manager is preferred. Must maintain current CPR certification throughout employment.
Office 365 computer skills (Microsoft Word, Excel and Outlook) along with some experience with a generic Healthcare Management System is required.
Bilingual (English/Spanish) is preferred.
Starting salary $70,000-$75,000 depending on experience.
**Please note that the working hours are Monday-Friday 8 am-5 pm ARIZONA time. If you live in a different time zone- you will still be required to work Arizona time so please make sure that works for you and your family prior to applying.
