Position Description
The Foster Care Manager oversees and supervises children in out-of-home care and works directly with the children, their families, and other relevant individuals. This position is responsible for the development, implementation, and review of service plans for the children and their families. The Foster Care Manager works in coordination and collaboration with the DHHS and the Courts, and other service providers. This position will implement strategies to alleviate barriers so the permanency goal can be achieved. This position works in several counties.
Reporting Relationship:
This position reports directly to the Foster Care Supervisor and is under the general supervision of the Child Welfare Director.
Responsibilities:
Engage and conduct thorough assessments to identify each individual and family's needs.
Develop individual treatment plans for youth, their families, foster families, and/or other caretakers to ensure safety and well-being in achieving permanency.
Identify and make appropriate referrals for relevant services.
Complete reports and quarterly assessments regarding progress of the case and overall family functioning.
Attend all court hearings, DHHS meetings, and all relevant case-related meetings.
Meet all deadlines and complete all required paperwork to ensure compliance with MDHHS policies and contract, and DCWL rules regulations.
Conduct minimum monthly face-to-face home visits with clients in foster care and meet with foster parents/relative caregivers.
Coordinate and schedule parenting time visits per policy and court-ordered expectations.
Coordinate and ensure sibling visitation occur monthly.
Coordinate monthly meetings with birth parents as required by policy, and at least quarterly meetings in the parental home/residence.
Meet monthly with biological parents as required by policy, review referral services, and assist in the integration of services provided the client/family.
Ensure medical and dental services for all foster children are scheduled and maintained by the caregiver; monitor the implementation of these services by the caregiver.
Provide emergency/on-call interventions after hours, and on holidays/weekends, as needed.
Fulfill all other case management services for assigned clients and families as required by MDHHS policy and all other duties as assigned; maintain an average caseload of 10
Requirements:
Education: Bachelor's degree in Social Work or relevant Human Services field: Sociology, Psychology, Forensic Psychology, Education, Community Development, Behavioral Science, Gerontology, Special Education, Education of the Emotionally Disturbed, Education of the Gifted, Family Ecology, Community Services, Family Studies, Family and/or Child Development, Counseling Psychology, Criminal Justice, Human Services, a Human Services-Related Counseling Major, or Interdisciplinary Studies in Social Science.
Experience: Prior experience working in a child welfare agency is preferred.
Excellent written and verbal communication skills, excellent organizational, analytical, and interpersonal skills.
Able to work a flexible schedule to meet the requirements of the case, with "regular" business hours set by the Agency, including weekends and holidays, and as needed.
Able to perform on-call services 24-hours per day, seven days per week to assist agency consumers and/or provide crisis intervention as required based on needs of the case.
Experience working with children and families with extensive history of past trauma.
Access to a personal vehicle with liability insurance for work use at all times
Maintain a personal cell phone for client contact.
Other Requirements:
Prior experience with Microsoft Office and related applications.
Valid unrestricted driver's license.
Reliable ongoing personal transportation.
Personal Vehicle Safety Check.
Personal/Company Vehicle form completion with personal vehicle insurance information.
