Overview
Private Duty LPN – PRN (One Shift Every Other Week)
Location: Paris, TN
Overview:Adoration Home Health & Hospice is seeking a skilled Licensed Practical Nurse (LPN) for a PRN position providing one-on-one in-home care. This role requires G-Tube, trach, and ventilator support, offering a flexible opportunity with one 12-hour shift every other Sunday.
Schedule:
- PRN: One 12-hour shift every other week (Sunday)
Responsibilities:
- Deliver personalized care, including G-Tube, trach, and ventilator management.
- Monitor and assess the patient’s condition to ensure safety and comfort.
- Collaborate with the care team to provide high-quality care tailored to the patient’s needs.

Certifications Required:
- Active LPN license in Tennessee.
- Current CPR certification.
- Minimum of one year of nursing experience required due to ventilator certification training.
Responsibilities
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
