POSITION TITLE:SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM (SNAP) EMPLOYMENT AND TRAINING PROGRAM (E & T) CASE MANAGER/COORDINATOR
QUALIFICATIONS:
- Certified Arkansas Teacher License
- Demonstrated proficiency in Google Suites and Microsoft Office.
- Successful candidates must have a commitment to working cooperatively with other satellite personnel to fulfill the Mission of the PCSSD Adult and have the ability to deliver quality customer service.
REPORTS TO: Coordinator of Adult Education
JOB GOAL: To coordinate program to identify clients’ needs and be able to refer clients to supportive services to include Adult Education.
PERFORMANCE RESPONSIBILITIES:
- Process referrals received from SNAP E&T Central Office on a weekly basis
- Follow the guidelines in the DHS SNAP E&T Handbook
- Conduct a preliminary assessment of individuals in a timely manner to TABE, Scale Scores and Educational Functional Level
- Provide reverse referrals for individuals when necessary
- Serve as participant case coordinator and help in the development of an individual education/career plan.
- Complete an Employment Plan for each participant and update as needed
- Provide job search assistance
- Monitor participants' progress monthly and provide steps to help maintain successful progress
- Maintain up-to-date participant files
- Identifies barriers faced by participants and develops a solution
- Complete all required reports in a timely manner
- Must complete Career Coach Certification within a year after hire
- Other duties and special projects as assigned
TERMS OF EMPLOYMENT:
Salary: $25.00 per hour
Length: As Needed
FLSA Status: Non-Exempt
Personnel Status: Certified
Benefits eligible: Yes (30 hours required to be eligible for benefits)
EVALUATION:
Performance of this job will be evaluated according to provisions of the Board’s policy on Evaluation of Professional Personnel.
