The Direct Support Professional staff, under the direct supervision of the Senior Case manager, Day Habilitation Supervisor, Manager, and Program Director is responsible for the following:
- Ability to drive a 15-passenger van.
- Assist with transporting individuals to and from program and designated sites for day (transportation may involve using public transportation).
- Implement Person Centered Planning using an Interdisciplinary Team approach that will help those individuals he/she is serving in becoming more integrated within the community.
- Ensure compliance regarding agency (ARC), state (OPWDD), City (Dept. of Mental Health, Mental Health & Alcoholism Services, Dept. of Labor (DOL) and Medicaid Waiver regulatory guidelines.
- Implement Day Habilitation Service Plans and complete daily service notes reflecting services and individual’s response.
- Collect individual’s Data for the Day Hab. Plan by utilizing accumulate records, direct implementation and observation, and other significant input.
- Attend case conferences and training, when necessary and appropriate.
- Ensure the safety of the individuals at all times.
- Provide crisis intervention on an as needed basis. This includes First Aid and CPR (Once trained and certificate has not expired).
- Report all incidents and minor occurrences to your supervisor and provide proper documentation (i.e., employee statement).
- Must be able to work independently and as part of a team; maintaining clear communication with management, guardians/caregivers (as needed), vendors, and members of the individual’s support team.
- Maintain the integrity of the physical environment and agency materials/property (i.e., agency vans, phones, computers/laptops). Ensure to communicate any safety and/or health hazards or potential hazards on, as well as off, site immediately.
- Immediately inform your supervisor of any issues and complications to your license.
- Teach and model appropriate workplace culture to the individuals.
- Create and maintain an enthusiastic and educational atmosphere.
- Uphold the rights of the individuals and adhere to the policies and procedures related to their rights (i.e., HIPPA).
- All other functions deemed appropriate and within the scope of this position.

Schedule:
- 8 hour shift
- Monday to Friday
Qualifications
- High School Diploma or GED;
- Preferable experience working with individuals with developmental/intellectual disabilities
- Drivers License REQUIRED.
Skills Required:
- Must be able to follow and carry out written and oral instructions.
- Must be able to read, write and speak the English language in an understandable manner.
- Must possess the ability to make independent decisions and handle stressful situations when circumstances warrant such action.
- Must be able to lift and carry up to 50 pounds
- Must possess the ability to deal tactfully with the Individuals, Personnel, Family Members, Visitors, Government agencies and the General Public.
- Must be willing to work harmoniously with professional and non-professional personnel.
- Must have the ability to implement the programs, goals, objectives, policies, procedures, etc. of the Residential Services Department.
- Must have the patience, enthusiasm and be able to handle the Individuals based on the level at which they are currently functioning.
Experience:
- Caregiving: 1 year (Preferred)
License/Certification:
- Driver's License (Preferred)
