Description

The Regional Director is responsible for the direct oversight of the Assistant Directors of Care Management. The Regional Director is a key leadership position responsible for supervision of a Care Management team, with a focus on continuous quality improvement in alignment with ACA/NY’s quality indicators. This position will support implementation of agency wide policy and procedures to ensure standardized practices are upheld throughout ACA/NY’s Care Management program. The Regional Director is expected to liaise with local Developmental Disabilities Regional Offices (DDROs) and provider agencies. The Regional Director will partner with Staff Development in the development and delivery of training curriculum to support Care Management. This position is an on-call position.

 

ACA/NY is a 501(c)(3) not for profit organization that has been designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. ACA/NY is dedicated to meeting the needs of people with Intellectual and Developmental Disabilities by providing comprehensive care management and coordination of service. ACA/NY supports 25,000 people in its program with support services spanning New York City, Long Island, and the Lower Hudson Valley.

 

Duties & Responsibilities

Care Management Leadership

  • Ensure the provision of timely and responsive care management services to all enrolled members.

  • Coordinate with other Regional Directors to plan for new enrollments.

  • Monitor disenrollment’s and CCO transfers to ensure timely completion by the Assistant Director.

  • Contribute to the development of ACA/NY’s strategic plan, with a focus on quality outcomes. Participate in and facilitate agency wide workgroups as needed.

  • Collaborate with ACA/NY Senior leadership to develop actions plans and project manage work plans, to achieve strategic initiatives.

  • Collaborate with Staff Development and Senior Leadership in developing and implementing identified trainings.

  • Liaise with the local Developmental Disabilities Regional Offices (DDROs) and provider agencies. Represent ACA/NY at formal speaking engagements, community events, panel representations, and other identified events within the designated region.

 

Supervision

  • Support employee growth through regular coaching and mentoring, using person specific performance goals.

  • Serve as a point person for the Director of Administrative Support to coordinate procurement, IT and furniture needs across ACA/NY’s 8 Hubs.

  • Track service utilization, using data analytics and metrics, to maximize service delivery to members supported.

  • Monitor caseload capacity to ensure caseload optimization and efficiency. Oversee intake and discharge planning, to ensure caseload assignments meet weighting requirements.

  • Monitor schedules and work hours for all reports, through the daily review and approval of employee timecards and final payroll approval of bi-weekly payroll, within the required timeframe.

  • Track PTO requests of all reports to ensure adequate coverage. Review, evaluate, and approve/deny PTO requests from Assistant Directors.

  • Ensure incident reporting guidelines are upheld and work with ACA/NY’s Quality Assurance department to implement immediate safeguards.

  • Attend and facilitate department/team meetings, trainings, support meetings, in accordance with agency practice and policy.

  • Complete all required trainings and ensure all direct reports adhere to training requirements.

  • Implement recommendations, as the result of any investigations, within required timeframes.

  • Address underperformance, as necessary. Develop and deliver progressive discipline when appropriate, in collaboration with senior leadership and human resources.

  • Evaluate the need for new positions within the program and participate in talent selection. 

 

Quality Assurance

  • Responsible for the overall management and implementation of ACA/NY’s Quality Management Plan for assigned region.

  • Monitor quality indicator outcomes and provide support as needed to ensure those outcomes are met.

  • Audit Care Management records to ensure compliance and quality standards are met. Ensure identified findings are reported and corrective measures are implemented, to improve quality outcomes.

  • Facilitate continued performance improvement by developing systems to support the Care Management teams to meet desired outcomes.

  • Maintains confidentiality in accordance with HIPAA and privacy practices.

 

Additional requirements

  • 24/7 agency-wide, rotating on-call responsibilities required for agency.

  • Responsible for being accessible by phone outside of regular business hours including holidays and weekends to respond to and attend to member emergencies under scope of oversight.

  • Travel as needed to support agency initiatives and member needs.

  • Perform other duties as assigned.

Qualifications


 

  • Bachelor’s degree with 5 years of relevant supervisory experience or a Master’s degree (preferred) with four years relevant supervisory experience.
  • QIDP status required (BA/BS in a health and human services field of study 1yr I/DD Experience or MA/MS in a health and human services field of study)
  • Prior experience within the provision of care management preferred
  • Absolute sense of integrity and personal commitment to serving people with I/DD and their families;
  • Excellent interpersonal, public speaking, and written communication skills;
  • Ability to work autonomously;
  • Demonstrated leadership abilities;
  • Demonstrated professionalism, respect and ability to work in a team environment;
  • Sound knowledge and proficiency in the ability to utilize technology and an electronic health record.

Work Environment

This is a remote position with a blend of work from home, field work, and regional travel as well as in office expectations.  Approximately 85% of work time will be remote, with the remainder being in-office.  Whether working from an ACA/NY office, one’s home, or another remote location, each employee is required to follow minimum privacy standards when working in the flexible work environment:

  • Employee has access to a password secured WiFi connection and/or ACA/NY Mobile Hotspot.
  • Employee is able to secure any printed materials containing individual or employee personal and/or health information, so as no one else in the household will be able to access it.
  • Employee is able to conduct phone and video calls in a private area where the conversation cannot be overheard, and their screen is not visible to others.
  • Employee is able to conduct their work in a location where personal and health information on a monitor is not visible to others.

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinet, etc.  The noise level in the work environment is usually moderate. Travel to off-site locations is required; therefore, the incumbent must be comfortable driving or using various forms of public transportation to reach destinations.

Physical Requirements

While performing the duties of this position, the employee is regularly required to perform the following: Engage in verbal and written communication, operate a computer, smartphone, and other office tools and equipment; travel between locations and within locations, stand, walk, sit, reach with hands and arms; stoop, kneel, and crouch, moving objects up to 25 pounds, and to visually or otherwise focus and navigate; ACANY will provide reasonable accommodation, in accordance with applicable policies and laws.
