Position Summary

The Assistant Director of Care Connection is responsible for supervising a Regional Care Connection team, throughout our network. In conjunction with the Director of Care Connection, the Assistant Director of Care Connection is responsible for meeting key performance indicators related to Medicaid eligibility, collection of required documentation and timely enrollment of new members. The Assistant Director is responsible for direct oversight of Care Connection Specialists.

ACANY is a (c) not for profit organization that has been designated as a Care Coordination Organization / Health Home (CCO / HH) by New York State. ACANY is dedicated to meeting the needs of people with Intellectual and Developmental Disabilities by providing comprehensive care management and coordination of service. ACANY supports 25, people in its program with support services spanning New York City, Long Island, and the Lower Hudson Valley.

Duties & Responsibilities :

Admissions Activities

  • Coordinate and oversee all activities related to new member enrollment in assigned regions, including scheduling interviews, document collection, and verification of Medicaid status, incompliance with OPWDD regulation.
  • Assign enrollment interviews with potential new members to Care Connection Specialists.
  • Provide referral information for those who require a different service model.
  • Ensure Admissions Coordinators complete all necessary documentation and / or facilitate new assessments with agency service providers.
  • Support Director of Admissions to manage capacity and maintain current vacancy by region, in cooperation with Care Management Regional Directors.
  • Participate in planning activities with the Care Management team, and shares recommendations for ensuring seamless enrollment
  • Provide education to families on the OPWDD Front Door, including access to benefits for new enrollees
  • Form and maintain working relationships with the OPWDD Regional Offices within ACANY’s network to ensure ongoing referrals and to address service needs identified.

Quality Assurance

  • Assist in tracking and reporting enrollment trends, providing regular snapshots to senior management that include recommendations of needed competencies and service gaps.
  • Audit Care Connection Specialists records to ensure compliance and quality standards are met. Ensure identified findings are reported and corrective measures are implemented, to improve quality outcomes.
  • Facilitate continued performance improvement by developing systems to support the Care Connection team to meet desired outcomes.
  • Supervision

  • Support employee growth through regular coaching and mentoring, using person specific performance goals.
  • Monitor schedules and work hours for all direct reports, through the daily review and approval of employee timecards and final payroll approval of bi-weekly payroll, within the required timeframe.
  • Track PTO requests of all reports to ensure adequate coverage. Review, evaluate, and approve / deny PTO requests from Assistant Directors.
  • Ensure incident reporting guidelines are upheld and work with ACA / NY’s Quality Assurance department to implement immediate safeguards.
  • Attend and facilitate department / team meetings, trainings, support meetings, in accordance with agency practice and policy.
  • Complete all required trainings and ensure all direct reports adhere to training requirements.
  • Implement recommendations, as the result of any investigations, within required timeframes.
  • Address underperformance, as necessary. Develop and deliver progressive discipline when appropriate, in collaboration with senior leadership and human resources.
  • Evaluate the need for new positions within the program and participate in talent selection.
  • Additional requirements

  • Complete all required trainings.
  • Attend department / team meetings, trainings, supervisions, etc. as scheduled and in accordance with agency practice and policy Completes all required trainings.
  • Travel throughout the designated service area to meet with people receiving services and their interdisciplinary team, as necessary.
  • Maintain confidentiality in accordance with HIPAA and privacy practices.
  • Perform other duties as assigned.
  • Qualifications

  • Bachelor’s degree in health and human services or licensed RN with 3 years of relevant experience or a master’s degree with 2 years of experience relative to the job duties outlined above. Working knowledge of OPWDD Front Door process, preferred but not required.
  • Absolute sense of integrity and personal commitment to serving people with I / DD and their families.
  • Excellent interpersonal, public speaking, and written communication skills.
  • Ability to work autonomously.
  • Demonstrate leadership abilities.
  • Demonstrate professionalism, respect, and ability to work in a team environment.
  • Salary : $73,000 - $75,000
