AdvanceCare Health Services, LLC is seeking to hire a compassionate and dedicated Program Manager. The Program Manager will oversee an Individual’s care to ensure all state and agency requirements are met, and records are kept up to date. This is a hybrid of work-from-home, coming to the Hermitage, TN office as the department needs require. Must have an excellent driving record. Must have high-speed internet.
JOB DUTIES:
- Ensure visits, staffing, and other plans are kept up to date
- Obtain and upload documentation and other items as required to maintain up-to-date records
- Perform site visits and audits on schedule
- Monitor and follow up on required drills and community visits as required.
- Provide Training including but not limited to pre-service and in-service training.
- Work with families, providers, and other agencies monthly
- Assist as needed with Incident management and corrective action committees
- Enforce standard company policies and procedures and ensure all are in compliance.
- Develop schedules for annual drills, appointments, meetings, etc., and provide reports to the director.
- Ensure all client files, records, plans, and lists are up-to-date. Maintained according to policy/ procedure.
- Meet the attendance requirements established by the Director.
- Works a flexible schedule, including long hours and overtime when needed.
- Performs other duties as assigned and needed.
SKILLS AND QUALIFICATIONS:

- Thoroughness and careful attention to detail.
- Ability to document and analyze documentation

- Ability to handle all situations with professionalism and a positive attitude and effectively communicate with individuals at all levels of the organization.
- Knowledge of office computer applications and Microsoft Office suite of applications (e.g., Word, Excel, PowerPoint) and/or business management software).
REQUIREMENTS:
- High school diploma or GED equivalent, with caregiving and office management experience is preferred.
- Experience in a healthcare or corporate environment is preferred.
- Satisfactory criminal background check.
