What are the responsibilities and job description for the Chart Abstractor position at Advanced Cardiovascular Specialist?
We are seeking a detail-oriented and experienced Chart Abstractor (Temp) to join our team for a limited-term project. The Chart Abstractor will be responsible for reviewing medical records and extracting key clinical data according to project-specific guidelines and quality standards. This role is essential to ensuring accurate, consistent data collection to support quality initiatives, reporting, or registry submissions.
Key Responsibilities:
- Review and abstract data from electronic and/or paper medical records.
- Extract required information such as diagnoses, procedures, lab results, treatments, and outcomes.
- Enter abstracted data into designated databases or software systems with a high degree of accuracy.
- Follow project protocols, coding standards, and documentation guidelines.
- Communicate with clinical or data quality staff when needed to clarify documentation.
- Meet productivity and quality benchmarks consistently.
- Maintain patient confidentiality and comply with HIPAA and other relevant regulations.
- Participate in training sessions and team meetings as required.
Qualifications:
- High school diploma or GED required; associate or bachelor’s degree in Health Information Management, Nursing, or a related field preferred.
- Experience with medical record review and data abstraction, preferably in a clinical or registry environment.
- Knowledge of medical terminology, ICD-10, CPT coding, and clinical documentation.
- Familiarity with EHR systems such as Epic, Cerner, Meditech, etc.
- Strong attention to detail and analytical skills.
- Ability to work independently and meet deadlines.
- Certified Tumor Registrar (CTR), RHIT, RHIA, LPN, RN, or other relevant credential preferred.
