Director of Patient Care Services (RN) – Home Health
Are you an experienced Registered Nurse (RN) with a passion for leadership and home health care? Advanced Care Home Health is seeking a dedicated Director of Patient Care Services (DPCS) to help lead our team and shape the future of our growing home health agency in San Luis Obispo County, CA.

What You’ll Do
As the DPCS, you’ll be responsible for overseeing patient care, ensuring compliance with state and federal regulations, and leading our clinical team with expertise and compassion. Your key responsibilities will include:
- Supervising clinical operations and ensuring top-quality patient care
- Developing policies and procedures to maintain compliance and excellence
- Leading, training, and supporting a team of nurses and healthcare professionals
- Collaborating with physicians, patients, and families to create effective care plans
- Ensuring compliance with state and federal home health regulations including Title 22, Medicare COP and CHAP
- Perform nursing case management visits as necessary
Qualifications:
- A Registered Nurse with a Bachelor’s Degree in Nursing or other health related field with three years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity; or a Registered Nurse with four year experience within the last five years in home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity in a home health agency.
- Knowledge of relevant federal and state regulations, and guidelines for current home health assessments and care classifications.
- Must have excellent observation, verbal and written communication skills.
- Must be a passionate self-starter that thrives in a team environment. Must have strong critical thinking and problem-solving skills and must have excellent attention to detail with an eye on the big picture.
- Must have excellent interpersonal and conflict resolution, mediation, and crisis management skills, with the ability to exercise good judgment and confidentiality.
- Ability to maintain comprehensive working knowledge regarding information systems and applicable software programs like electronic medical records.
- Must be available for on-call duty outside of office hours per arranged schedule.
- Must pass health screening for TB clearance, physical examination, and background check.
- Must be able to travel via personal vehicle for the performance of job duties. If driving must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage.
