What are the responsibilities and job description for the Developmental Disabilities Specialist position at Advanced Care?
Advanced Care LLC is currently hiring for a Life Skill Coach "Specialized Habilitation Associate". Our company is dedicated to helping adults and kids with developmental disabilities thrive and complete their personal goals. The main purpose of this position is to prevent our customers from being institutionalized.
You will work one-on-one with adults with developmental disabilities to design a specific, measurable, achievable, and relevant plan which addresses their individual goals. Goals will fall under these five categories:
- Self-Empowerment
- Safety Awareness and Self Advocacy
- Interpersonal Effectiveness and Effective Social Communication
- Coping Strategies for life challenges, and managing daily tasks and acquiring new skills.
- You and the client will use a form to develop goals and develop a plan of action.
- You will need to drive to the client's location to provide services in person.
Requirements: All requirements must be met.
- A bachelor’s or master’s degree in Psychology, Sociology, Social Work, Education, Child Development, Gerontology, or Nursing.
- Minimum of 2 years or more working with adults with developmental disabilities.
- Knowledge and ability to teach coping skills, communication skills, using frequency charts and comparing data, developing accessible plans, using reinforcements, and assessing a customer’s needs.
- Must provide unofficial or official transcripts of your degree.
- Must provide references where you worked with adults with developmental disabilities.
- Must be able to work in the United States.
Responsibilities
- Monitor and document progress, ensuring compliance with care plans and goals.
- Engage in ongoing training and professional development related to special education and behavioral health.
- promote inclusion in the community and be individualized to the Client participant. provided with the intent of reaching a habilitative goal identified in the PCSP and in compliance with the Specialized Habilitation Policy. provide support to address up to three goals under one (1) or more of the following Specialized Habilitation domains as authorized by DDA:
