Responsibilities:
The home health aide is responsible for providing personal care, including baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, and assisting with toileting activities. They also plan and prepare nutritious meals, assist in feeding patients if necessary, take and record temperatures and vital signs when ordered, assist in ambulation and exercise according to the plan of care, perform range of motion and other simple procedures as an extensional therapy service as ordered, assist patients in self-administration of medication, do laundry as appropriate, meet safety needs of patients, and use equipment safely and properly. Additionally, they report on patient condition and significant changes to the assigned nurse.
Essential Job Functions/Responsibilities:
The certified home health aide must adhere to the organization's documentation and care procedures and standards of personal and professional conduct. They must submit all clinical documentation, including Oasis, within 24 business hours of visit.
Position Qualifications:
To be eligible for this position, candidates must meet the training requirements in accordance with state and federal laws, be at least 18 years of age, have the ability to read and follow written instructions and document care given, possess empathy for the needs of the ill, injured, frail, and impaired, maintain current CPR certification, demonstrate tact, patience, and good personal hygiene, and have a valid driver's license. Candidates must also be able to work independently and within a multidisciplinary team.
