Job Responsibilities
- Oversee the implementation and ongoing assessment of patient plans of care.
- Work closely with the IDG team to ensure accurate evaluations and comprehensive care planning.
- Communicate patient conditions and collaborate with providers to deliver high-quality care.
- Provide supervision of aides and nurses.
Required Skills and Qualifications
- At least one year of experience practicing as a Registered Nurse (RN) in a hospice or similar setting.
- Ability to work closely with the IDG team.
- Strong communication and leadership skills.
