Key Responsibilities:
- Oversee the implementation and ongoing assessment of the patient's plan of care.
- Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
- Evaluate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
- Provide supervision of aides and nurses.

Requirements:
- One year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.
- Ability to relocate to West Mifflin, PA prior to starting work.
